Our service in aged care

At Forteture Health Group, we understand that as people age, their healthcare needs become more complex and require a multidisciplinary approach. Our goal is to optimize the physical and mental health of the elderly, whether it is rehabilitation after a fall, improving mobility, managing pain, or enhancing mental health and wellbeing. Our services are designed to promote independence, dignity, and a better quality of life for our clients.

That’s why we provide a range of allied health services in aged care to help improve the health and wellbeing of our elderly clients. Our team leader was deeply inspired and influenced by the advanced rehabilitation care structure in Japan (Power Rehab©) and integrated it into our care model for the elderly in the community. We work collaboratively with aged care facilities and their staff to provide tailored care plans that meet the specific needs of each client, and we also travel to clients’ homes under the HCP scheme.

What is HCP ?

HCP, or Home Care Package, is designed to help elderly live independently in their homes for longer. It is a government-subsidized program that provides personalized support and care services tailored to the specific needs of each individual. With a home care package, seniors can receive a range of services, such as help with daily tasks, personal care, and allied health services. The goal of home care package is to empower seniors to remain in control of their lives while receiving the assistance they need to maintain their independence and dignity. This allows them to continue living in their homes and participating in their communities, surrounded by the people and things they love.

Conditions that we can help you with

Physiotherapists can help improve your pain, balance, strength, movement, pain, swelling, physical function, injuries to nerve and soft tissues, manual handling, transfer, and mobility issues from acute or chronic conditions

Occupational therapists can assess your ability to perform daily activities (e.g. showering, eating and toileting), prescribe equipment and aids to assist with certain functions, conduct pressure care, train your fine movement control, and optimise cognitive abilities

Massage therapists can help to reduce pain from deconditioned muscles, and joints, particularly from chronic pain caused by conditions such as arthritis. They can also improve circulation, increase flexibility in the soft tissues, reduce stress and anxiety through their soothing techniques, and promote better sleep through various relaxation techniques.

Exercise physiologists commonly work with elderly who have chronic conditions such as diabetes, heart disease, chronic obstructive pulmonary disease (COPD), arthritis, osteoporosis, and chronic pain. They help to design and implement exercise programs tailored to each individual’s specific needs and goals, considering their medical history, physical limitations, and fitness levels.

Dietitians work with elderly clients to educate on healthy diet and develop individualized meal plans that address their unique nutritional needs and health concerns. as well as help with meal planning, grocery shopping, and food preparation. Common conditions in elderly that are seen by dietitians include diabetes, heart disease, high blood pressure, malnutrition, obesity, digestive disorders, food allergies and intolerances, and swallowing difficulties. 

Podiatrists commonly work with elderly clients who have foot and lower limb problems such as arthritis, diabetes, poor circulation, nerve damage, and skin conditions. They provide preventative care, treatment, and management of foot and lower limb problems, including nail and skin care, wound management, orthotics, and footwear advice. They also work closely with other healthcare professionals such as doctors, physiotherapists, and occupational therapists to provide holistic care to their clients.

Speech pathologist commonly work with elderly clients who have speech, language, communication, and swallowing difficulties. Common conditions include: aphasia (difficulty in communicating), dysphagia (difficulty with swallowing), voice disorders (such as breathiness), cognitive-communication disorders (memory, attention), Parkinson’s disease, and dementia.
