What is NDIS

At Forteture Health Group, we are passionate about helping people of all abilities lead fulfilling lives. That’s why we offer a range of services to help individuals with disabilities achieve their goals. To attend our services, you need to be an NDIS participant. The NDIS is an initiative by the Australian government designed to provide support and funding to eligible individuals aged between 0-65 years with disabilities. If you or a loved one is eligible for NDIS funding, Forteture Health Group can help.

Our process

If you already have a support coordinator: you can attend an initial consultation and complete a service agreement if you’re happy to come on board. We’ll take care of everything else, including setting up your profile, creating a treatment plan, booking all appointments and assessments, and reviewing your goals if needed.

If you are new to the NDIS: simply fill out our ‘Enquiry & Refer’ form below. We will connect you with a potential support coordinator to discuss your case and contact you for a possible assessment.

Special program run with our partners

We work with our partners to diversify the experience of our clients through offering them different activities and events:

The training sessions are held in the Olympic Park every Monday afternoon.

Piano & Music appreciation

Our partner is a leader in the music education industry, with a focus on excellence in classical piano training in Sydney. This research-based piano school offers a serious approach to learning and hosts concerts throughout the year.

Outdoor

All kinds of outdoor events are organized throughout the year.

 FAQ

At Forteture, most of our therapy appointments are booked in for 1 hour which includes 50 minutes of face-to-face therapy and 10 minutes of admin time for the therapist to complete session notes and plan your next session. However, it varied depending on the client’s needs.

We follow NDIS guidelines. Currently a 24 hour notice needs to be given to avoid cancellation fees. These fees are deducted from your NDIS funding.

A participant's NDIS Plan funding is intended to last for the entire period of the plan. However, we understand that situations can arise and funding may be used up much sooner than anticipated. If this happens, our team can assist in lodging an NDIS Plan Review well before the end date, if appropriate.
